All applications may be filed electronically through the eServices link below. Paper applications are provided below for Tobacco, Nicotine, and Vapor Products (TNVP) retailers. Applications not completed through eServices should be mailed to: 500 Mero Street 2NW21, Frankfort, KY 40601. Please make checks or money orders payable to Kentucky State Treasurer.
